WHAT IS HIRSUTISM?
Hirsutism means increased body hair of male type in women. It means a coarser, dark pigmented hair that is typically localized to the upper lip and chin, around the nipples, in the middle line of the lower abdomen, the back of the abdomen, and groin and thighs. This type of hair is stimulated by male hormones (androgens).
Who gets hirsutism? Polycystic ovarian syndrome (PCOS) is the most common cause of hirsutism and explains up to 70-80% of cases. PCOS, in turn, is a very common hormonal disorder in women of childbearing age and involves an increased activity of androgens. The condition has a hereditary component but is also dependent on lifestyle factors that affect body weight (overweight).
However, in about 20% of patients with hirsutism, the androgen levels in the blood are normal. An explanation for hirsutism can then be an increased sensitivity in the skin to male sex hormones. Some rare conditions such as hormone-producing tumors and other specific hormonal diseases can also cause hirsutism. Furthermore, some medications and abuses of anabolic steroids can increase body hair.
How are the symptoms developed? Symptoms of increased body hair often begin in connection with puberty and can then gradually increase until the early 20s. Continued development is largely dependent on body weight. With heavy weight gain, body hair often increases. Other symptoms associated with hirsutism in many cases are sparse menstruation and acne as in PCOS. In rare cases, hair loss also occurs centrally on the head. If the symptom onset is not typical, the hair growth is pronounced and has increased rapidly and any other symptoms have occurred, unusual hormonal conditions should be suspected.
How is the diagnosis made? Body hair can be graded and followed according to a developed assessment scale (Ferriman & Gallwey), but usually the diagnosis is based on a general assessment with regard to ethnic differences in body hair and pigmentation. The study includes analyzing the androgen content in the blood and, if necessary, investigating menstrual disorder with, among other things, ultrasound to investigate the presence of polycystic ovaries as in PCOS. In case of suspicion of unusual conditions such as hormone-producing tumor, specific investigation is required.
How are patients treated with hirsutism? At overweight, weight loss is recommended because weight loss leads to improved hormone balance with lower androgen levels. The same is seen after a smoke stop. First-hand medication in hirsutism in younger women is estrogen-contraceptive contraceptive pills that have the effect of inhibiting male sex hormone. In case of insufficient effect of birth control pills or when the contraceptive pill is inappropriate, the patient may be offered so-called antiandrogen medication which can sometimes be more effective.
